	Copy of Last Year’s Tax Return
	W-2’s
	Record of Other Income (1099’s)
	Interest and dividends from savings accounts, money markets, mutual funds, stocks, & bonds
	Sale of stocks, bonds and mutual funds:  date acquired and cost; if dividends were reinvested, all year-end statements
	Other income including social security, lottery winnings, unemployment, jury duty.
	Rental property income and expenses.
	Copies of all K-1’s (Partnerships and S-Corporations)
	Pension or IRA distributions.
	IRA contributions.
	Child care- provider’s name, address and social security number.
	College tuition

	Moving expenses (when move is due to change in jobs)
	Purchase, sale or refinancing of residence
		Closing papers from purchase and sale of old residence
		Closing papers from purchase of new residence
		Closing papers from refinance of current residence
		Copy of tax return showing purchase of old residence
		tax preparation fee for prior year
